Program : Community Support Services, Geneva, NY

Shift Schedule : Monday - Friday 8:00am – 4:00pm or 8:30am – 4:30pm

Salary : Salary pay range is min. $17.52 to a max. $22.82 per hr. based on education & experience

    Job Summary

    With the general oversight of the CSS Program Manager, works from a trauma informed care perspective to perform a wide range of recovery-based services including but not limited to support, advocacy, linkage, and coordination of services to support engagement with behavioral health providers and collaboration between members of the treatment team. Individual will be located within the County Mental Health Counseling Center and will strive to support a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences.

    Essential Job Functions:

  • Coordinate and participate in weekly treatment team meetings

  • Provide outreach calls and visits to high risk clients

  • Provide outreach calls and visits to clients not following through on appointments

  • Gather collateral information to ensure collaboration and communication amongst team members

  • Make calls to collaterals to discuss client concerns and updates

  • Assist in coordination of urgent medical assessments for those who are not lined with a primary care physician (PCP)

  • Assist in linkages to local Federally Qualified Health Centers (FQHC)

  • Assist in coordination of transportation

  • Ensure timely follow up with PCP

  • Conduct home visits to evaluate home environment when there are concerns for safety/self-care

  • Assist with linkage to benefits to include but not limited to:

  • Medicaid/Medicare

  • SSI/SSD

  • Public Assistance

  • Link individuals with community resources

  • Ensure “warm hand off” when making referrals to other services

  • Utilize service dollars (when available) to assist eligible individuals (SPMI) in gaining access to or supporting participation in services where there is no other means of support for that access or participation

  • May work with both the adult and children & youth populations

  • May assist in transporting clients to urgent services as needed.

  • Assist the crisis team as needed.

    Education and Experience:

    Two (2) years of experience in providing direct services to people suffering from a mental illness or

    One (1) year of case management experience and an associate’s degree in a health or human services field or

    One (1) year of case management experience and an additional year of experience in other activities with people suffering from a mental illness or

    Bachelor’s degree or master’s degree which includes a practicum encompassing a substantial number of activities with people suffering from a mental illness. Valid NYS Driver’s License (as driving is an essential function)

    Knowledge:

    Thorough knowledge of community services within service area; general knowledge and understanding of Mental Illnesses, Psychiatric Rehabilitation model and related issues; skills for independent living.
