Indian Health Service Social Worker - DE in Gallup, New Mexico

Summary The position is located with the Gallup Indian Medical Center, Division of Clinical Services, Social Services branch in Gallup, New Mexico. The purpose of the position is to provide professional social work, discharge planning and case management activities. The work will require direct patient intervention, assessment, planning, coordinating and monitoring services with the various existing Federal, Tribal, State private and other local service providers. Responsibilities Exercises mature, professional judgment and flexibility in handling a wide range of casework skills in the study, diagnosis and treatment of patients and their families who present an unlimited range of difficult health related psycho-social, mental health, environmental and other problems. Determines independent conclusions of isolated and vulnerable patients who have complicated and conflicting needs regarding medical conditions/decisions, treatment plans, resources and relationships. In the functions of case management, will conduct initial case assessments, develop individualized service plans that will identify programs and services appropriate to meet patients' needs; access, coordinate/facilitate appropriate community and or other services, monitor services Makes independent professional decisions and recommendations for patient care planning regarding hospital discharge of patients to home or alternative living, community based or contract care rehabilitation or long term care. Independently prepares and interprets case studies and histories of patients for physicians, health care teams and community social and other agencies for their use in reaching decisions for further medical or psycho-social treatment. Prepares patient referrals and coordinates services for patients with other professionals, community agencies/resources such as social service, education, law, judicial and health organizations. Qualifications To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): Degree: Social work. Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council on Social Work Education. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: GS-11: Three (3) years of progressively higher-level graduate education leading to a Ph.D. degree or Ph.D. or equivalent doctoral degree; or one (1) year of specialized experience equivalent to at least the GS-09 level. This experience is related to the work of the position and equivalent to at least the GS-09 level in the Federal service as described in the vacancy announcement. Examples include: collaborating with other health professional staff to assess client needs; refer clients, patients and families to community resources; and provide individual or group counseling. SELECTIVE PLACEMENT FACTOR: Incumbent must possess a current valid unrestricted license U.S. State license as a Licensed Clinical Social Worker (LCSW) or a Licensed Medical Social Worker (LMSW). This credited service can be used in determining the rate at which they earn annual leave. This position is covered by a Bargaining Unit. No government housing.
