Responsibilities: * Preforms visits and completes coordination of client care. * Operates as the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. * Consults as needed with the physician and the office giving details about patient care. * Performs accurate OASIS collection, ensures the medication profile remains current, and ensures lab values have been reported to the physician in a timely manner. * Attends weekly case conference and monthly case manager meetings. Required: * Must hold a current and unencumbered Registered Nursing License in the state which you intend to work. * Current state driver\'s license Preferred: * 1 year of clinical experience.
