Trinity Health Lead Case Manager - Full Time in Fresno, California

Description:

Reporting to the Manager CareCoord&SocalSvcs (INACTIVE), Social Services, this position is responsible for assisting in the execution of the Utilization, Quality, Care Coordination and Discharge Planning program within the hospital, and will address clinical and resource outcomes across the continuum of the Integrated Delivery System. This position assists to maximize the efficient delivery of care to hospitalized patients from the Emergency Department, ensuring both the appropriate use of hospital resources and timely sequencing of patient care events. This position will promote continuity of care for clinical patient outcomes across the continuum, and will reduce the use of unnecessary inpatient resource through effective assessment, plan development and implementation for patient disposition.

  1. Current licensure as a Registered Nurse in the State of California is required.

  2. Degree from an accredited baccalaureate program is preferred.

  3. Current certification in Case Management (CCM) is required within one year of hire into position.

  4. Previous experience in at least two areas of clinical specialty, effective communication skill, good evaluative, interpretive, and problem solving skills are required.

  5. Knowledge regarding hospital protocol and procedures, familiarity with community resources and outside professional agencies, familiarity with federal and state regulations governing hospital and home care, as well as understanding of the financial structure of health plan and delivery system is required.

  6. Leadership and collaborative skills relevant to consulting, advising and evaluating case management colleagues are required.

  7. Effective communication and teamwork skills are required.

  8. Current American Heart Association (AHA) Healthcare Provider CPR card is required.

Pay Range

$54.95 - $79.68
