The After Hours RN is primarily responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Qualifications: One to two years of recent LTC experience in an ALF/MC/SNF setting. One year of hospice care experience preferred. Experience and desire to perform direct patient care necessary Graduate of an accredited school of nursing Current Minnesota Nursing License On-call position: every other weekend 5pm Friday until 8am Monday, as well as 2-3 weeknights 5p-8a Participates in holiday rotation Strong phone communication skills Proficient with technology and technological applications: Microsoft Office, Smart Phone, Calendaring, Wi-Fi, EMR, Internet Browsing, Printer Contact Lifespark Now!
