Full-Time Nurse Practitioner - Queens

Responsibilities :
Involve with direct patient physical assessment, education, and health planning
Maintain continuity of care by regular patient follow-up
Assist in the coordination of services with other disciplines and patient's insurance plans
Advocate for patient in arrangement of medically necessary services
Communicate with community's resources to ascertain adequate assistance for patients in need
Refer patients to specialty services when necessary
Consult with collaborating physician and other members of the health care team to establish plan of care
Participate in quality assurance process to assure optimal care delivery
Provide comprehensive case management services that include emphasis on health promotion and disease prevention
Monitor patient outcome in accordance to the practice protocols
Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning
Act a resource person for the nursing staff
Conduct education in service on practice issues
Perform other duties as assigned
Minimum Qualifications :
Graduated from approved Nurse Practitioner Program
Licensed as Nurse Practitioner in New York State
Basic CPR (BCLS) certification required
At least 1 to 2 years of clinical experience as a primary care Nurse Practitioner
Board/Specialty Certification preferred
Knowledge of a second language (such as Mandarin, Cantonese, Korean, Spanish) preferred
Compensation and Benefits :
Salary: $105,000.00 - 115,000.00 per year and p roductivity-based incentive pay
