Family Resource Center of Northwest Ohio, Inc. Wraparound Facilitator in Findlay, Ohio

Be a part of facilitating a process that allows for youths and families voices to guide and design the actions of an established team, taking necessary action to ensure the safety of clients. Assist in the development of a team composition that reflects unique youth and family strengths and needs. Under direction of the Clinical Supervisor, you will facilitate plans that act in the best interest of youth clients and families to address outcomes across home, school, and community through one synchronized plan. This facilitation will be provided to youth referred by the Family First Council Service Coordinator. Conduct an assessment of community support needs for each client and develop a plan accordingly. Ensure that the team is dedicated to and has the capacity to design and implement a comprehensive plan over time. Assist in crisis situations, record client progress, monitor relapse plans, maintain necessary documentation, and report progress to probation/parole. Partner closely with other agencies and programs in order to cultivate working relationships, ensure adequate client care, provide consultative service and/or promote the coordination and development of mental health and/or substance abuse services. Complete referrals to other agency programs or external service providers as deemed clinically appropriate. You have.

  • HSD/GED
  • A valid driver's license with the ability to meet minimum required level of auto insurance per carrier guidelines, as well as the willingness to transport clients in your personal vehicle as needed
  • CPR/First Aid Certification or willingness to obtain one
  • Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR and other online systems, with multiple logins often simultaneously.
  • Willingness to submit background check and fingerprinting

Preferably.

  • Bachelor's degree or higher in Social Work, Psychology, Human Services, or closely related field
  • Experience working with youths and families across multiple systems
  • Experience providing case management or crisis services work
