The primary purpose of your job position is to coordinate and schedule external appointments for all participants at

the PACE Organization (PO). The scheduler will prepare schedules according to the participant needs and the

participant's care plan designed by the interdisciplinary team. Works closely with the transportation department,

participants, caregivers, and center staff.

  • Position Title: Scheduler Location: LIFE St Mary, Feasterville-Trevose PA Hours : Full Time Position Purpose: The Scheduler will coordinate and schedule external appointments for all participants at the PACE Organization (PO). This role involves preparing schedules in line with participant needs and care plans as designed by the interdisciplinary team, and working closely with the transportation department, participants, caregivers, and center staff. What You Will Do:

  • Demonstrate the Mission, Vision, Values, and Guiding Behaviors of Trinity Health and Trinity Health PACE (TH PACE) in all interactions.

  • Create and maintain a warm, calm environment in the office.

  • Collaborate across all divisions within TH PACE.

  • Adhere to established policies, procedures, and work schedules.

  • Foster a team environment and resolve conflicts.

  • Maintain knowledge of relevant laws, regulations, and organizational policies.

  • Be prepared for disaster recovery and crisis management as needed.

  • Attend required training sessions.

  • Maintain accurate participant schedules and update them as necessary.

  • Communicate effectively with all departments regarding participant appointments.

  • Ensure accuracy in scheduling and participant education.

  • Utilize electronic scheduling systems and track agency personnel training.

  • Review and approve agency invoices in the manager's absence.

  • Create necessary authorizations and remind participants/caregivers of appointments. Minimum Qualifications:

  • High School Diploma or equivalent required; Associate's Degree preferred.

  • At least one year of scheduling experience.

  • Experience working with a frail or elderly population is preferred.

  • Compliance with health requirements as specified by state regulations.

  • Strong business acumen, conflict management, customer focus, and decision-making skills.

  • Excellent written and verbal communication skills.

  • Proficiency with Microsoft Office Suite and scheduling systems.

  • High degree of personal accountability and independent decision-making ability.

  • Superior organizational skills and ability to manage multiple tasks.

  • Problem-solving skills and flexibility in work priorities.

  • Ability to travel up to 10% within the center's geographic region. Position Highlights and Benefits:
