Farmingdale State College invites applications for a Case Manager position within the Campus Mental Health Department. Under supervision, the case manager provides comprehensive case management for the chronically challenged and/or societally struggling in order to improve quality of life and maintain the highest possible ability to function within the College community. Works to facilitate care by assessing student needs, evaluating on and off campus resources, coordinating care, and assessing progress. Interacts with physicians, social workers, families and human services providers. Seeks to improve clinical outcomes, increase student support, performance and well-being; and promote cost-effectiveness.

Duties and Responsibilities:

  • Coordinate and facilitate student care through assessment, evaluation, planning and implementation.

  • Work collaboratively with students, families, health care providers, and student services professionals to ensure high quality care.

  • Act as the student's advocate as it relates to insurance coverage, financial assistance, housing and health status.

  • Facilitates client access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services; assists client to develop natural resources and contact social support networks.

  • Coordinates and monitors services, including comprehensive tracking of client activities in relation to care plan.

  • Assists clients in developing goals and areas of need, and assists in personal growth plans which are assessed regularly.

  • Documents all client encounters and contacts made on behalf of clients; completes and submits billing documentation as appropriate; maintains comprehensive client files, which may include documents held for safekeeping on behalf of the client.

  • Assesses students, evaluates effectiveness of care plan and progress made by client, participates in student treatment planning and case review with student care providers.

  • Maintains and reports applicable statistics regarding programs and client services.

  • Ongoing advocacy for community, state, and legislative change; remains aware and prepared for grant opportunities.

  • Organizes and manages a needs assessment.

  • Attends monthly SUNY Homeless Liaison meetings

  • Performs miscellaneous job-related duties as assigned.

    Job Requirements:

MINIMUM QUALIFICATIONS:

  • Bachelor's degree.

  • Two years of clinical or related experience.

  • Ability to gather data, compile information, and prepare reports.

  • Knowledge of community health care and vocational services.

  • Ability to plan, implement, and evaluate individual student care programs.

PREFERRED QUALIFICATIONS:

  • Master's degree in Nursing, Social Work, Mental Health Counseling or a related field.

  • Four years of clinical or related experience.

  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.

  • Strong ability in networking, liaison, interpersonal skills and social service resources.

  • Records maintenance skills.

    Additional Information:

This is a full-time UUP position.

  • For the first 10 working days, beginning Thursday, August 29, 2024, and ending Thursday, September 12, 2024, applications will be limited to UUP Farmingdale.

  • CLOSING DATE FOR RECEIPT OF APPLICATIONS: Friday, September 27, 2024

  • SALARY: $55,000 + $3,400 in downstate location pay = $58,400 Total Compensation.
