Job Location - Sunrise Community Medical - Evans, CO Salary Range - $15.00 - $17.23 Hourly Job Shift - Day Monday, Tuesday, and Friday: 9:00 am - 1:00 pm Wednesday and Thursday: 1:00 pm - 5:00 pm

Program Overview: Integrated Care Integrated Care - North Range recognizes that wellness is based on the interconnection between physical, mental, emotional, and spiritual health and approaches individuals with each of these components in mind. Integrated care allows for better continuity of care through improved communication and care coordination between primary care and behavioral health providers. Behavioral health services include assessments, therapy, crisis management, and care coordination.

Peer Support Specialists are people who have been successful in the recovery process and who help others experiencing similar situations. Through shared understanding, respect, and mutual empowerment, peer support specialists work to help people become and stay engaged in the recovery process and reduce the likelihood of relapse.

Position Summary: With a Collaborative, Customer First, and Compassionate approach, the Peer Support Specialist will: Embrace the North Range Behavioral Health Mission Statement - Provide compassionate, comprehensive care for people who face mental health and addiction challenges. Provide a supportive environment to help clients identify their own goals toward recovery and or/resiliency. Teach skills to work toward the client's self-identified life goals (social, educational, housing, benefits, illness management, etc.) Secures resources and services for clients with mild to severe mental illness, disorders, and disabilities. Coordinates with internal and external supportive resources to deliver needed services to clients. Collaborate with team members regarding services in response to the treatment plan.

Minimum Qualifications: High School Diploma and/or ability to successfully demonstrate reading, writing, and speaking skills. A Valid Driver's License and driving record that meets the hiring requirements as set forth for someone who must transport clients/program participants is required. Must have personal experience or been impacted by mental illness, substance abuse issues, other co-occurring challenges, or child and family issues. Must be 1-year free of drugs and/or alcohol and demonstrate working a recovery program. Successful completion of substance use disorder treatment must be 1- year from completion of such treatment. No current or recent legal involvement within the past 2 years. Association with the recovery/peer support movement and/or participation in a peer or 12-step support/mentoring program or setting such as Narcotics Anonymous, Alcoholics Anonymous, or Celebrate Recover preferred.
