WorkSource Oregon PGH Health Services Coordinator in Enterprise, Oregon

The Health Services Coordinator (HSC) is responsible for coordinating resident health care and overseeing medication administration under the direction of the Residential Administrator and the Registered Nurse (RN). The HSC acts as a medication aid when needed. Qualifications: * Valid driver\'s license. * Pass an employer criminal history check and drug screening. * High school diploma or equivalent preferred. * Certified Nurses Aid Certification or Medication Aid Certification, or willingness to obtain your medication aide certification Or, a combination of experience and education in similar types of roles preferred. * Two years of experience providing direct patient care and medication administration preferred. * Willingness to work morning shifts, swing or graveyard shifts at any residential treatment facility as needed. Duties: * Train medication aids under the direction and supervision of a Registered Nurse. * Works collaboratively with the resident\'s care team, including pharmacists, psychiatrists, therapists, case managers, guardians, and medical providers. * Schedule resident medical appointments and rides. Attend offsite and in-house appointments with residents if other staff are not able to attend. * Report medication discrepancies, medication administration, compliance issues and resident needs to RN and/or Administrator. * Manage and approve new orders including monthly cycle fill according to current policy and RN instruction. Administer prescribed medications. Report needs to RN or Administrator. * Document in a timely, concise, and accurate manner, per policy, by completing monthly progress notes. File documents into resident charts, including scanning into the electronic documentation system. * Participate in intake and discharge procedures at the Administrator\'s instruction. * Be available for after-hours on-call consultation, when scheduled. * Manage residents experiencing crisis situations and provide guidance to staff during crisis situations. Report suspected changes in a resident condition and/or potential crisis situations to the RN. * Fosters a positive workplace environment that promotes retention of staff, and where staff input is encouraged, listened to, and responded to. * Additional duties as assigned.
