The Licensed Practical Nurse (LPN) is responsible to gather data concerning the resident's condition and as an integral member of the interdisciplinary team, use that data to drive the nursing care of residents within the skilled nursing setting. Responsible for ensuring the continuity of care of the patients/residents between shifts by providing direct care as well as supervising the care given by CNAs and supportive staff members. The LPN participates in resident and family teaching. Maintains role as resident advocate with a focus on our facility's mission. Provide direct and indirect resident/patient care activities of daily living including personal care, comfort, and maintenance ofa safe and clean environment for care facility residents. *Duties and Responsibilities Understand and adhere to White Pine Care Center's Corporate Compliance Standards, Code of Conduct, and all Department/Personnel policies and procedures. Report changes in the resident's condition to physician, Care Coordinator, and family on a timely basis. Document condition changes to resident's medical record and communicates change to next shift. Document accurately and concisely on admissions, transfers, deaths, and discharges. Fulfill requirements for documentation according to policy and as resident condition dictates. Collaborate effectively with all other disciplines in meeting needs of the resident. Directs the ongoing care of the resident on a daily basis during the assigned shift. Supervises and/or delegates appropriately to CNAs. Assesses the condition of the resident accurately and collaborates with the DON/ADON as indicated. Physically and mentally responds to emergency situations; able to initiate and delegate CPR. Administers medications and treatments as prescribed and assesses the effectiveness of these. Effectively handles and reports emergencies, accidents, incidents and complaints. Accurately receives, interprets and transcribes Physician orders. Stands and walks for extended periods of time.
