The Discharge Planner assists inmates/residents with medical or mental health issues in successful reentry into the community, ensuring continuity of care for chronically ill inmates/residents after discharge. This role communicates with external sources regarding inmates/residents on their caseload, including family contact.

• Participates in the intake process

• Collect, organize and analyze information about individuals through records, tests, interviews, and professional sources.

• Responsible for establishing goals, developing plans, coordinating service delivery and appraising the interests, needs, aptitudes, abilities and personality characteristics of inmates/residents for effective reentry.

Qualifications:

• Graduate from an accredited college or university with a Bachelor's degree in social work; or Registered Nurse (RN) with current RN licensure in the state of employment is required.

• Three (3) years of discharge planning experience is preferred.

• Experience in a prison/jail setting and/or in-patient medical facility is preferred.

• Experience providing case management services is preferred.

• Must demonstrate the ability to communicate clearly verbally and in writing.

• Experience with Microsoft Office applications or other similar software applications is required.

• A valid driver's license is required.

• Minimum age requirement: Must be at least 18 years of age.
