Walworth County Limited Term Public Health Nurse (HHS) in Elkhorn, Wisconsin

Seekingtwo full-time, limited term RN or LPN Public Health Nurse to provide services in public health programs that promote and maintain the health of individuals, families, groups, and the community. Positions are responsible for applying the nursing process and public health principles, interventions, and essential services including educating, counseling, preventing disease and promoting wellness. This position is required by Wisconsin DHS chapter 140. This is a limited term position that is anticipated to finishby the end of the year. Provide health assessments and educational services using the nursing process and essential public health services to individuals and groups using public health interventions based on best practices for the identified public health setting. Provide clinical nursing services for immunizations. Conduct epidemiologic investigations, surveillance and control of communicable disease as mandated by DHS Chapter 145 and 252 and case management, referral, and follow-up with consumers and providers as applicable. Participate in department and community emergency response training and drills in support of public health emergency and disaster preparedness consistent with job classification. Prepare and maintain Public Health Department records and reports in accordance with agency policies and State requirements. Respond to inquiries from the general public and provide technical assistance and consultation to community agencies and health care providers regarding communicable disease. Deliver targeted, culturally appropriate information to help individuals and groups understand health promotion and disease prevention, policies, regulations and local code. Adhere to ethical and legal principles and department policy in the collection, maintenance, use and dissemination of data and information. Demonstrate a commitment to county safety and risk management efforts. Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience Associate or Bachelors degree in nursing from an accredited program required with a minimum of six months of relevant experience, internships or clinical experience may qualify. Previous experience with a public health agency preferred. Bilingual in Spanish preferred. Proven experience in ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals as well as ability to compute rate, ratio and percent and to draw and interpret bar graphs. Knowledge of Trauma Informed Care principles is essential. Certificates, Licenses, Registrations Valid Wisconsin drivers license Proof of minimum liability insurance coverage Wisconsin Practical Nurse or Wisconsin Registered Nurse through WI Department of Safety and Professional Services. CPR certification Interactions and Communications Provides specialized information and/or recommendations to others regarding an area of expertise. Ability to work with multi-ethnic, multi-cultural populations.
