Adverse event reporting is one of the key strategies to identify risks and improve safety. This position leads and provides oversight of the adverse event reporting platform including day to day support to the enterprise for adverse event (AE) detection, processing, and reporting. Initiates, coordinates, and manages strategic risk reduction strategies throughout the HMH Enterprise.

Responsibilities

A day in the life of a Clinical Risk and Loss Prevention Specialist at Hackensack Meridian Health includes:

  • Collaborate with Executive, Regional, Local, Nursing and Clinical Leadership to create a sustainable culture of risk prevention to reduce harm for both patients and team members (inclusive of all caregivers); improve the human experience and reduce exposure throughout the HMH enterprise.

  • Collaborate with Regional and local Campus Risk Management and Quality/Safety leadership to promote a culture of safety, a culture of reporting, and a Just Culture by supporting and encouraging non-punitive event reporting of adverse safety events.

  • Continuously optimize the Hackensack Meridian Health Adverse Event Reporting platform to meet the needs of stakeholders, working to create efficiency, and to yield high quality information.

  • Leverage the adverse event reporting platform as a tool for leaders to understand harm within their business unit.

  • Leads the governance structure to lead decisions and engage stakeholders in the creation of a reporting platform that meets at a minimum the needs of Enterprise Risk Management, the Department of Patient Safety and Quality, and the Office of Patient Experience.

  • Partner with data analytics to develop de-identified dashboards which allow for trended information.

  • Presents information at network and campus committees driving awareness on systems learning derived from speaking up and reporting.

  • Infuses mechanical learning, artificial intelligence and automation into the current system; while monitoring for unintended consequences and for quality assurance.

  • Addresses the factors that hinder reporting or are barriers; develops appropriate solutions, facilitating proactive involvement of stakeholders, and changes the overall reporting culture

  • Collaborates with site risk, patient safety/quality, patient experience, human resources leadership to respond to adverse events/grievances. Uses decision making ability to analyze and recommend disposition strategies. Oversee and direct occurrence reporting process for site. Ensures timely data entry of reports received. Completes necessary trend analysis of occurrence reports.

  • Develops a safety culture where reporting is encouraged and use of the Onelink adverse event reporting system is monitored, with interventions to address issues as needed.

  • Oversees risk management software to drive risk mitigation and safety culture improvement across the network; i.e. the adverse event reporting system (Onelink). Encourages a culture of reporting across the region.

  • Provide timely monthly reports to Sr. VP & Chief Risk Officer of activities at site.

  • Create and implement new programs to foster a culture of safety and safe environment for patients, visitors, team members, provides, volunteers, and customers of the facility.

  • Provide statistical analysis for review by appropriate committees as requested.

  • Collaborate with all stakeholders to optimize workflows for event reporting and analysis.

  • Design educational and training programs for the platform to drive clinical and operational risk reduction.

  • Envision, recommend and implement enterprise wide clinical risk guidelines, protocols and standards.

  • Develop Key Performance Indicators and Dashboard for Clinical Risk & Loss Prevention.

  • Provide litigation support to HMH Legal Team and external Defense Panel Counsel.

  • Serve as senior clinical risk advisor to the enterprise.

  • Leads or co-leads the Patient Safety Organization (s) for HMH.

  • Other duties and/or projects as assigned.

  • Adheres to HMH organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Minimum of 10 years of progressively responsible experience in the healthcare industry.

  • Minimum of 3 years of Healthcare Risk Management or Patient Safety experience required.

  • Strong understanding of the following:

  • Project Management

  • Enterprise Risk Management

  • Process Review.

  • Root Cause Analysis.

  • Department of Health and Joint Commission requirements familiarity.

  • Emergency Management.

  • Regulatory Compliance.

Education, Knowledge, Skills and Abilities Preferred:

  • Bachelor's degree in Nursing, Business Management or related health field.

Licenses and Certifications Preferred:

  • Risk Management Certification
