Fairview Health Services Admissions Coordinator II in Edina, Minnesota

We are seeking an Admissions Coordinator for our Ebenezer Corporate office in Edina, MN! This administrative position would be responsible for developing relationships with referral sources and working with internal teams to promote and maintain census of the transitional and long term care units. You would assist the Social Services Department in meeting the needs of the senior residents and families of the care center.

This position includes;

  • 80 hours in 2 weeks

  • Day shift

  • One weekend per month

Ability to process and complete move-ins:

  • Provides pre-move in counseling, information and tours

  • Follows intake process and secures primary providers

  • Reviews EPIC for proper orders and contacts pharmacy as needed

  • Determines needs for equipment, supplies and interpreter and coordinates with internal team

  • Coordinates admissions process and assures completion

  • Informs residents and residents representatives of financial responsibilities and verify payer sources

Promotes professional working relationships within Ebenezer/Fairview and outside referral sources

  • Assists with marketing & outreach activities as appropriate

  • Conducts outreach through campus and facility events

  • Tracks referral sources and other information as requested

  • Works with internal team to maximize facility occupancy

Ability to assist residents with various takes:

  • Assists residents with mail, finances, and MA as appropriate

  • Notifies residents and their representatives of room changes

  • Assists residents with ordering personal items

  • Offers support to residents and families and acts as advocate for residents

Ability to assist Social Service team with various tasks

  • Oversees care conference rescheduling

  • Tracks advance directives and legal appointment documentation

  • May participate in vulnerable adults investigations

Ability to participate in Vulnerable Adult investigations

  • Knowledge of the facility policy and regulations

Required:

  • Vocational/Technical Training or Associate Degree

  • Current MN LPN license

  • 1 -3 years of work experience

Preferred:

  • Bachelor's Degree in Medical or Human Service Field

  • 3-5 years of experience

  • 10 years' experience in LTC

  • Computer skills

  • Experience in Medical or Human Service Field
