HCR Home Care TFSS - Therapy Field Staff Supervisor - Central NY Branch in East Syracuse, New York

Role and Responsibilities

The Therapy Field Staff Supervisor (TFSS) is an integral part of the patient care team and continually monitors the rapy services provided . This role has shared responsibility for the overall provision and coordination of home health services provided to the HCR patients , according to acceptable standards of practice. The TFSS is a resource person with extensive knowledge of the regulatory guidelines governing home health agencies and provides daily support to field clinicians enabling them to provide quality care in the home to HCR patients.

The TFSS is a liaison between referral resources, patients, caregivers, and employees.

Essential Functions

  • Act as liaison to assist in appropriate ongoing management of care; ensure appropriate plans and interventions are identified , act as resource for problem solving or advising on patient care questions or concern .

  • Complete review of Physician ordered Plan of Care (POC)/485 and OASIS Assessment to ensure completeness and that they accurately reflect patients ’ condition and needs .

  • Complete review of Resumption of Care (ROC) and discharge assessment data and approved orders , as needed .

  • Review and complete all assigned HCHB - related tasks and coordination note workflow on a daily basis , based on set timeframes. Communicate with supervisor when unable to complete timely .

  • Enter new patient orders as appropriate and update the plan of care and patient calendar as appropriate .

  • Review and approve all new interim orders before they are sent to physician. Notify Clinical Manager if unable to approve the order .

  • Assure orders are written according to HCR protocol/regulatory requirements .

  • Verify/ E nsure there are orders for requested medical supplies .

  • Enter supply orders and requisitions, as needed .

  • Follow-up on orders from the Order Tracking Report when the MRS is unable to retrieve the unsigned order if requested .

  • Update pathways, as needed, to provide intervention and goals for documentation .

  • Review missed visit reports, ensure MD notification has occurred and , if not, notif y MD .

  • Ensure clinical documentation is present to support need for ongoing services .

  • Review and process vital sign alert reports including documenting any action, follow-up and physician notification .

  • Perform end - of - episode reviews , ensuring regulatory requirements are met .

  • Review and process discharges (discipline and agency) .

  • Entry of Hospital Hold Order/Note, if needed, including follow-up on patient status at hospital .

  • Follow-up with staff to correct or refine documentation that is not accurate or complete to ensure it standards of documentation are met .

  • Support staff by assisting with administrative clinical functions (i.e. , TIF’s, phone calls, etc . ) .

  • Alert Clinical Manager of changes in patient status or orders that do not seem appropriate . Conduct case conferences (informal or supervisory) to discuss patient issues and determine if patients that are planned for discharge meet the requirements for management and evaluation or any other skilled service , and document conferences in the medical record via the Case Conference Coordination Note .

  • Conduct Interdisciplinary Case Conferences according to agency protocol .

  • Receive calls from field staff, physicians, patients/caregivers, or others regarding care questions or issues.

  • Attempt to resolve any issues at the initial point of contact.

  • Enter information regarding all telephone calls into the medical record .

  • Notify staff, as appropriate , to ensure proper coordination of care .

  • Act as back-up for the PSC in rescheduling missed and declined visits , processing reassigned and reschedule requests to ensure timely completion of these tasks .

  • Provide support to the Intake staff and process referral requests, as needed .

  • Support the Insurance Specialist s ; may be required to verify benefits and obtain authorization for services on all payers requiring authorization , as a primary responsibility of the TFSS role .

  • Conduct in-home supervision of Therapy staff, as directed .

  • Create Aide Care Plans, when needed .

  • Assist with Telephonic Case Management, as needed .

  • Perform chart audits, when requested .

  • Adhere to, and participate in, the Agency’s mandatory HIPAA/Privacy Program and Employee Compliance Program .

  • Read and adhere to all Agency policies and procedures and follow the Employee Handbook Guidelines .

  • Other duties as assigned .

    Clinical Manager Possible Backup Duties

  • Enter non-admit information into HCHB with details in coordination notes , if no visit was made .

  • Review Recert/Discharge Decision; process/complete task .

  • Coordination Note review: POC Oversight, Biller Notification, HHABN, Late Visit Documentation .

    Education Requirements

  • Bachelor of Science Degree in Physical, Occupational or Speech required .

    Qualifications and Requirements

  • Current, valid Therapy license (PT/OT/ST) issued by NYS Department of Education required .

  • Three (3) years ’ experience in the health care industry with a minimum of 1 year experience in home health care.

  • Supervisory experience is preferred .

  • Understand issues related to the delivery of home health services and be able to problem solve effectively .

  • Possess knowledge of the Medicare/Medicaid guidelines governing home health agencies .

  • Organized, detail-oriented and possesses effective communication skills both verbally and in writing .

  • Able to work independently , but also enjoys functioning as a part of a team .

  • Excellent customer service skills .

    Work Environment

    The TFSS is primarily in an office setting and may be exposed to outdoor conditions.

    The working conditions are classified as sedentary work:

  • Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
