Description/Job Summary
Crossroads is hiring for a full time Blended Case Manager!
Snapshot of the Case Manager Position:
Community Based
Hourly Rate of $19.32 per hour (including an on-call stipend of $90)
$1,500 sign-on bonus! after completion of 180 days of employment
Monday- Friday 8am-4pm with rotating on call responsibility
Qualified Candidates will have a Bachelor's Degree in Human Services related field
Company Fleet Available
This is a community based position, working directly with adults diagnosed with a mental health diagnosis in the community to provide support in crisis plannning, accessing services, reaching goals and linking/referring to resources to community resources to maintain independence, stability and self-sufficiency. All employees are provided a cell phone and laptop along with access to company vehicle during working hours.

Responsibilities/Duties
Assessments
Goal Planning
Linkage and Referrals to community resources
Gaining access to services
Monitoring of services delivery
Crisis Planning and problem resolution
Informal support network building
Following all operational procedures and regulations
Assisting clients in attending community appointments
Moving clients into their residences
On-call after hours/weekends on a weekly rotation basis (on-call stipend included)
Required Experience
Bi-lingual and previous experience with case management and/or Mental Health/Drug and Alcohol desired.
Required Education
Bachelor's degree in a human service field.
Required Qualifications
Ability to pass a Background Check and Child Abuse Clearances
The ideal candidate manages multiple priorities, is flexible, and has excellent communication skills.
Must be able to bend, stand, and sit, for long periods of time and lift up to 25lbs.
Must possess a valid driver's license, car insurance and a registered/inspected vehicle and live with a 30 minute radius (approximate) due to on call responsibilities.
