Key Responsibilities:
Collaborate with hospitals and medical staff to assess patient needs and facilitate placements
Review medical records and patient history to make informed decisions
Work closely with case managers and hospital staff for seamless transitions
Complete admissions processes and provide ongoing support to admitted patients
This is not a desk job - traveling to facilities in the area is required!
