Moore County Hospital District Clinic Social Worker - Full Time 8am - 5pm in Dumas, Texas

This is a Full-Time Position. WHAT YOU'LL DO:   SUMMARY:  Responsible for providing professional services to patients/residents and their families related to health and social needs in the following service areas: Primary Care Clinics, with cross training to Acute Care, Long Term Care, and Hospice.  Collaborates with other community agencies/facilities as needed at the District level and for population health endeavors.    The social service employee works in collaboration and continuous partnership with patients, family/caregivers, healthcare providers, and community resources to: * Promote timely access to appropriate care * Increase comprehension through culturally and linguistically appropriate communication and education * Connect patients to relevant community resources with the goal of enhancing patient health, increasing patient satisfaction, and reducing health care costs The social worker is expected to serve as a contact point, advocate, and informational resource for patients, care teams, payers, and community resources.  He/She will conduct one-on-one extended patient meetings as well as follow-up with patients, family/caregivers, healthcare staff, and community resources as needed via secure email, phone calls, and/or other communications.  The social worker is expected to assess patients for unmet health and social needs, monitor for adherence to care plans, evaluate effectiveness, evaluate progress, and facilitate changes as needed.  EDUCATIONAL REQUIREMENTS: Sufficient to obtain licensure by the State of Texas in Social Work. Bachelor or Masters of Social Work Required.  EXPERIENCE REQUIREMENTS: One year of Social Work experience preferred. Proficiency in communication technologies (email, cell phone, fax, etc.)  Highly organized with ability to keep accurate notes and records.  Local knowledge about community healthcare and social welfare resources.  LICENSURE / CERTIFICATION REQUIREMENTS: Current Texas Social Worker License Texas Drivers License OTHER REQUIREMENTS: Proof of current auto insurance.  Core values consistent with patient and family centered approach to care.  Demonstrates appropriate and tactful communication skills.  Positive, respectful, and professional customer service.
