Matrix Human Services Resource Navigator in Detroit, Michigan

JOB SUMMARY/ACCOUNTIBILITY:

The Resource Navigator facilitates the coordination of community services rendered. to clients; provision of concrete services; collaboration with team members and community partners: and advocacy tasks, such as assisting in making appointments and securing financial benefits. This position requires a significant amount of paperwork.

 

GENERAL RESPONSIBILITIES:

  • Consultation with professional staff.
  • Eligibility - determine client eligibility for services; gather information from all relevant sources; develop a person-centered plan for service and intervention based on client needs and choices.
  • Assess/Reassess -- conduct initial and follow up in-depth assessment of client's overall status. 
  • Linking/Coordinating -- identify resources needed by the client, determine client eligibility, procure vocational and support services based on client needs and choices, assist client in applying for services, maintain a working knowledge of community programs pertinent to the client population and apprize administrative staff of unmet community needs. 
  • Case Documentation -- record progress notes and statistical information detailing 
  • pertinent activities and services.
  • Monitoring -- on-going face to face contact with clients to assure the effective provision of services and monitoring clients' progress toward treatment goals and objectives.
  • Perform accurate and timely completion of clinical and statistical records of staff interventions and activities.
  • Coordinate all group learning classes, including financial literacy, as needed by the program, and directed by administration.
  • Appropriately handle confidential records utilized internally and forwarded to other agencies for client assistance.
  • Maintain releases of information and requests for client records from outside sources.
  • Conduct activities in support of the team to provide program consistency.
  • Effectively maintain files.
  • Perform other duties as required.

     

POSITION QUALIFICATIONS:

  • AA/AS/ASW and 2 years' experience in human services preferred
  • Experience working with clients in challenging personal financial situations, including histories of incarceration, substance abuse, domestic violence and homelessness.
  • Michigan Driver's License
  • Excellent writing skills
  • Ability to handle multiple tasks
  • Ability to work effectively with and maintain patience, optimism and empathy with the population served
  • Ability to work in an interdisciplinary team setting with diverse populations.

     

WORKING AND ENVIRONMENTAL CONDITIONS:

Works in a normal office environment. Because of multiple sites within the agency and the need to meet with others within the community, it is common to drive to several locations, sometimes in inclement weather
