We provide comprehensive clinical solutions that position client facilities as the new gold standard in the skilled nursing and long term care industry.  Job Description The Social Worker's responsibility is to coordinate the services provided to geriatric residents and their families by the Clinical Social Workers (CSWs) and Psychiatric Advanced Practice Providers (APPs) by using tablets provided by the company for virtual care sessions.  The social worker will also be responsible for developing and monitoring weekly calendars of resident services by MPAC Healthcare clinicians, adding new admissions to the company EMR, and initiating insurance verification.  The social worker will participate as an active member of the interdisciplinary team which includes physicians and nurse practitioners.  The interdisciplinary team is responsible for a high level of organization which is necessary to establish, implement and evaluate the success of the program. The social worker will also work alongside the facility social service department and nursing staff to facilitate positive relationships between MPAC Healthcare employees and the facility. The social worker will participate in behavioral meetings and quarterly treatment plans, as per request.  The social worker will also interact directly with the residents and develop rapport, observe/report behaviors and provide insightful feedback to the MPAC clinical team.  Must also develop an understanding of the long term care environment.
