Description:

MercyOne Des Moines is looking for a Full Time Navigator RN Lead to join the team!

The Registered Nurse –Navigator Lead works autonomously and collaboratively with physicians, staff and other health care professionals, to develop, implement, and evaluate integrated and comprehensive plans of care for patients. Ensures best practice and high-quality standards of health care are maintained across the continuum of care by all members of the department.

What You'll Do:

  • Proactively acts as a patient advocate, working to overcome patient barriers, and resolve concerns.

  • Establishes and maintains a knowledge base of Medicare preventive screenings including any updates.

  • Investigates problems and makes recommendation to the manager for resolutions.

  • Assists with coordination of and participates in orientation and staff development activities as requested.

  • Utilizes staff to evaluate and determine priority situations by observing the staff within their assigned clinics.

  • Participates effectively as a lead team member in the clinic by being a resource and accountable to peers, providers and patients and acts as a liaison between the other staff and the manager.

  • Assumes responsibility for implementing QI with guidance from the manager.

  • Assists with implementing process changes or other duties as assigned to meet department goals.Maintains practice standards regarding quality improvement.

Minimum Qualifications

  • Possession of a current or compact state license as a Registered Nurse issued/defined by the State of Iowa.

  • Minimum of two years’ experience working in the AWV nurse role.
