Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness and/or co-occurring substance use disorders. The Center for Life Management (CLM) provides the area's most comprehensive array of mental health services. Our Integrated Care Program is an innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. Working together as a multi-disciplinary team, internal (CLM) and external (Lamprey Healthcare) work as one entity to support individuals with comorbid health conditions. The Behavioral Health Visiting Nurse is responsible for annual and quarterly assessments, treatment plans and nursing services to improve whole health outcomes for clients of the Integrated Care Program. This is a full time, 35 hour per week position. The schedule for this position will be Monday - Friday, 8:30am - 4:30pm.

Major Functional Areas and Description of Duties

  • Provide individual diagnostic assessment to determine client’s strengths, needs, resources, and goals;

  • Formulate treatment specifying presenting problems, objectives, interventions, and desired outcomes;

  • Complete assessments for eligibility determination for State funded services;

  • Provide nursing support services in the office, home or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan;

  • Take vitals regularly and communicate results with medical providers;

  • Provide education on chronic disease self-management;

  • Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement;

  • Provide Family Training and Counseling (FTC) services as needed to educate natural supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals;

  • Provide Medication Training and Support (MTS) services as needed to support clients in maintaining their medication regimen, as prescribed, integrated behavioral tailoring strategies into daily routine to promote effective medication management;

  • Provide crisis management and intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community;

  • Use clinical tools, data, and other information to complete nursing assessments of individual needs;

  • Coordinate medication refills and administer intramuscular injections as ordered by prescribing medical staff;

  • Provide set hours of billable service each week (eligible for incentive pay upon exceeding productivity expectation);

  • Provide timely documentation on each service delivered;

  • Work collaboratively with treatment team, primary care physicians, specialists, guardians, and natural supports as directed by prescriber to ensure continuity of client care;

  • Serve on appropriate committees, both internally and externally, as determined by supervisor;

  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients;

  • Execute all required services and documentation in accordance with agency policies and federal/state standards;

  • Maintain professional memberships, licenses, accreditation, certifications, etc.;

  • Adhere to Center’s Code of Ethics;

  • Represent CLM in a professional and appropriate manner in all settings;

  • Perform all other duties as assigned by supervisor.

    Qualifications, Skills and Experience

  • Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of New Hampshire;

  • Minimum of two years LPN or RN experience preferred;

  • Experience working with co-occurring disorders preferred;

  • Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical and medical situations;

  • Proficient computer skills, specifically with all Microsoft office applications;

  • Strong documentation skills and prior experience utilizing electronic records;

  • Strong ability and desire to work as part of an integrated clinical and medical team while also able to be self-directed and motivated with minimal supervision;

  • Ability to acquire and effectively practice new skills as needed according to evolving agency needs/services;

  • Ability to demonstrate effective communication skills, organizational skills, and ability to multi-task.

    Physical Demands and Working Conditions

The work will be performed in various settings including office based, visiting clients in the community and also in their homes. Manual dexterity is needed to use computers and applicable medical equipment. The ability to sit and stand for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.
