Griffin Health Ambulatory Care Pharmacist - Full Time in Derby, Connecticut

Location - 201 Division St. Derby, CT

Main Function:

  • Responsible for filling & refilling of all patient orders during the assigned shift. Delegates responsibilities to ensure patient care is #1.

  • Contacts/confers with physicians with questions/irregularities regarding prescriptions. Reviews & interprets prescriptions using patient profiles to ensure that overdoses have not been prescribed and that incompatibilities/drug interactions will not result from combining ingredients. Confers with physician to adjust drug doses based on renal & hepatic functions, patient age and specific disease states.

  • Inspects pharmacy inventory to ensure that drugs, which are outdated and/or have lost their potency, are removed from stock and replaced. Assumes responsibility to maintain adequate inventory by placing orders when needed.

  • In coordination with the primary care director, patient support leaders, director of operations, and systems team leaders, synthesizes quality measure closure progress from internally generated quality reports.

  • Serves in an educational capacity for patients and staff. Provides support to Medical group staff relating to Pharmacy procedures and drug information such as availability, toxicology, investigational uses, etc.

  • Maintains responsibility for attendance/reliability to ensure that the Medical group is operated in an efficient and cost effective manner.

  • Contributes to the mission of the Organization by supporting the Employee Philosophy, Planetree Model of Care, and organizational goals.

Education:

  • B.S. degree in Pharmacy.

Experience:

  • State of CT registration/licensure required.
