Specific Responsibilities:

This is a Senior Licensed Practical Nurse position working in the Delray Beach Health Center. Work is performed under the supervisor of theSenior Community Health Nursing Supervisor. This position requires independent judgement, initiative, applying general knowledge of business practices and skills in applying instructions to accomplish various job functions.

This position is designated as a sensitive position is required to maintain confident maintain confidential information in accordance with the Florida Department of Health (FDOH), Palm Beach County (CHD) Information Security Policy, Protocol and Procedures. Confidential data sets include Social Security, Clinical information data. This position will view, update and release information from, Health Management System (HMS), Pharmacy Forms System (P.F.S), and EMDEON. HMS - View, update, and release information -Client scheduling module for patient/clinic schedules, service collection, billing- service collection lab. Laboratory Test Collecting and Tracking. Pharmacy Forms System (PFS) and EMDEON - View, update, and release information – critical system to dispense by nurse issuance.

  • Maintain a perpetual inventory and audit trail for every medication in the clinic to ensure accountability.

  • File prescriptions, daily logs, offer to counsel signature logs, invoices, transfer documents, etc.

  • Place orders with Central Pharmacy through the PFS system

  • Receive orders and stock shelves.

  • Organize and maintain patient specific medication area:

  • Log medications and verify accuracy of invoice and actual receipts

  • Remove drugs that have not been picked up and return them to Central Pharmacy

  • Place order for refills with Central Pharmacy

  • Check stock medications for expiration dates and remove any outdated items from active storage area and places them in the quarantine area

  • Maintain quarantine log

  • Package and prepare documents for transferring medications between clinics

  • Conduct inventory according to guideline

  • Maintain an adequate supply of all forms related to drug dispensing / issuance

  • Maintain stock of most commonly used Patient Prescription Information (PPI) sheets

  • Issue clinic medication supplies to appropriate areas of clinic and verify documentation is correct before replacing working stock in clinic areas

  • Provides coverage to the medication room at the Annex which services HIV and TB

  • Follow up of patients having recent placement of Long Acting Removable Contraceptives (LARC’s).

  • Verify laboratory and radiology studies are completed as ordered and results have been entered into the medical record.

    Select and deliver medications for dispensing/issuance to practitioner/nursing staff. Give Patient Specific Medications to patients and execute the related paperwork. Create and maintain a tickler file for Patient Specific Medication delivered to the Health Center and prepare items for return to Central Pharmacy, wholesaler or reverse distributor and does associated paperwork. Make appropriate entries in PFS and print out reports as required and Certified Mail. Maintain patient profiles offer to Counsel Signature Logs, and other documents to Central Pharmacy as required. Keep current permits framed and posted in medication room. Assist staff during inspections by regulating agencies. Performs clerical duties as required which includes but are not limited to data entry, answering the telephone, photocopying, filing and record keeping.

    Responsible to verify appropriate reports are on the patient chart prior to the visit. Obtain authorizations for visits. Assist in posting maternity patients after visits with provider; ensures appointments and referrals are made. Participates in patient reminder calls and follow up calls for missed appointments.

    Cross trained to cover the duties at the Annex which include HIV and TB medications ordering, issuing and providing assistance with the X-ray protocol.

    Perform other duties as assigned.

    Required Knowledge, Skills, and Abilities:

  • Advanced to intermediate computer skills and knowledge of computer programs (Microsoft Word, Excel, E-mail, internet, Electronic Health Record)

  • Ability to establish and maintain effective working relationships, customer service skills,

  • Ability communicates effectively to convey information (verbally and writing) to others.

  • Ability to plan, organize, and coordinate work assignments.

  • Ability to retrieve, interpret and disseminate health information data.

  • Ability to document per CHD guidelines in patient’s Electronic Health Record.

  • Ability to document history in patient chart.

  • Knowledge of nursing principles, techniques, and scope of practice of the Licensed Practical Nurse

    Qualifications:

    Licensure/registration/certification requirements (If applicable, list the appropriate Florida Statute or federal regulation cite):

  • This occupation requires licensure as a Practical Nurse in accordance with Chapter 464, Florida Statutes or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27.

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

  • A Valid Drivers License

  • This position might travel to another Health Center.
