Nuvance Health Clinical Pharmacy Specialist - Transitions of Care in Danbury, Connecticut

Summary:
The primary functions of the Transitions of Care Clinical Pharmacy Specialist are to work in collaboration with providers, perform medication reconciliation, counsel patients upon discharge, conduct post discharge follow ups, and complete other duties as assigned. The goal of this role is to optimize guideline directed care,providepatient medication education, optimize therapeutic outcomes, and address adherence barriers prior to hospital discharge.
Responsibilities:
Work with other health care professionals including physicians, care managers, nurses, and others to coordinate care at discharge for individual patients. These activities may include patient medication education, assistance with outpatient prescription coverage, and managing meds to beds.
Initiate conversations with patients and caregivers to ascertain their understanding, health literacy, beliefs, attitudes, goals, values, and preferences.
Assess the overall status of the patient’s health condition and evaluates medication profile for optimal patient outcomes.
Monitors therapy and goals of care.
Provide a reconciled medication list for patients prior to discharge.
Documents all activities and communication as appropriate.
Participates in the education and training of pharmacy residents and students, and serves as a role model and mentor.
Responds appropriately and immediately in emergency situations.
Maintain professional knowledge and competence. Ensures compliance with regulations to maintain accreditation and licensure.
Performs others duties as assigned.
Other Information:
Bachelor of Science degree in Pharmacy from an approved college of Pharmacy. Doctor of Pharmacy (PharmD) degree preferred.
Registered as a licensed pharmacist or eligible for reciprocity in the State of Connecticut. (Must obtain a registered license within 3 months of employment.)
Demonstrated knowledge and compliance with all State, Federal, and malpractice laws of pharmacy practice.
Previous clinical pharmacy practice experience (3 to 5 years) in institutional environment and/or completion of an ASHP-accredited hospital (PGYI) residency.
Possess the personal characteristics of professionalism, credibility, commitment to high standards, innovation, flexibility and accountability.
Excellent communication and interpersonal skills and experience working on interdisciplinary initiatives across all levels of organization.
Working Conditions:
Manual: Some manual skills/motor coord & finger dexterity
Occupational: Little or no potential for occupational risk
Physical Effort: Sedentary/light effort. May exert up to 10 lbs. force
