Essential Job Functions Overview
With the general supervision of the Care Management Program Manager, works from a trauma informed care perspective to provide support, advocacy, linkage, and coordination of services in a care management program for persons with mental illness and/or chronic health conditions, who qualify for Health Home services as designated by the Department of Health. This position will also provide a variety of administrative, coordinating, and monitoring functions. Individual should strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences.
Essential Job Functions
· Will carry a small caseload (up to 10 HH+ individuals) and follow the guidelines of the HH+ Care Manager Job Description
· Provide individual counseling, training and assistance
· Actively participate in required staff meetings, community meetings such as SPOA, in-service trainings and other continuing education opportunities
· Complete training for the HARP Assessment within first month of hire and any ongoing related trainings as they arise
· Identify HARP eligible/enrolled members, perform the HARP Assessment per guidelines, and collaborate with care managers, providers and Managed Care Organizations (MCO’s)
· Update plans of care based on the results of the assessments, track, and review minimally on an annual basis
· Participate in supervision and clinical supervision as required
· Review plans of care and provide supportive feedback to the Care Management team
· Complete discharges for enrolled members
· Assist with the referral assignment process
· Develop and maintain appropriate and accurate records and files in accordance with all county and organization policies and procedures as well as all governing and regulatory standards
· Assist Program Manager and Director with administrative responsibilities and provide support to the Care Management team
· Maintain regular and effective communications with supervisor, county service providers and all relevant parties as needed
· Know and adhere to all DOH/HHUNY/OMH/other agency regulatory policies and procedures
· Assist with engagement of potential members by outreaching referrals and going through the intake and enrollment process
· Become a Champion Trainer for the team and assist in the training process for new staff
· Complete audits as they come in and review the results with the management team
· Travel to remote offices to provide staff with support, guidance and training as needed
· Participate in on-call rotation
Note: This position differs from the Care Manager description in that:
Incumbent will be responsible for completion and administration of the HARP Assessment.
Incumbent has more administrative and coordinating responsibilities.
Experience, Education, & Physical Qualifications
A bachelor’s degree in a human services field and two (2) years of experience providing direct services to persons with mental disabilities, developmental disabilities, alcoholism or substance abuse; a bachelor’s level of education and five (5) years of experience working directly with individuals with Behavioral Health diagnoses; or must be approved for HARP qualification waiver (Internal applicants only will be considered for the waiver). Valid NYS Driver License (as driving is an essential function of this job)
Knowledge:
General knowledge and understanding of mental health issues, chemical addiction and the Psychiatric Rehabilitation model; psychology, personality, and group dynamics; principles and practices of conflict and crisis management, adult learning, motivation, interpersonal communications, the Human Services system; use of motivational interviewing to engage individuals; positive philosophy toward wellness/recovery and trauma informed care. Individual should strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences. General knowledge of chronic health issues and the impact they have on overall well-being.

Physical Requirements and Working Conditions: Substantial amount of driving involved. General office environment.
