POSITION SUMMARY

  • As an advanced practice provider (APP), you will work closely with our sleep medicine physician in the clinic. Sleep Medicine providers prescribe therapies, including medications and medical device therapy such as CPAP for apnea.

    The APP will see patients for follow up visits after their initial consultation and after sleep studies have been interpreted by the sleep medicine physician. The APP will work closely with physicians and various allied health staff on a multi-disciplinary team by providing comprehensive assessments, ordering, and interpreting diagnostic tests, formulating a diagnostic impression, developing, and implementing a treatment plan, monitoring the effectiveness of therapeutic interventions, and prescription of pharmacological agents and non-pharmacological therapies. They will also provide primary care services including assessing, diagnosing, prescribing, treating, and educating patients.

    POSITION QUALIFICATIONS

  • Education and Experience:

  • If Nurse Practitioner, must be a graduate of an accredited school of nursing with successful completion of an approved Nurse Practitioner program. Minimum of 2 years of experience working independently with patients in family practice, acute care or similar specialty. Experience with program growth preferred.

  • License/Certificates:

  • Current DEA certificate or Minnesota Nurse Practitioner License.

    ESSENTIAL RESPONSIBILITIES

    • In clinical collaboration with the attending physician, provide day to day organizational and clinical direction and leadership of the healthcare team.
  • Perform comprehensive history and physical assessment for patients scheduled to service/practice setting.

  • Work with physicians to achieve service specific outcome targets for key metrics including clinical outcomes, and patient satisfaction.

  • Participate in performance improvement activities aimed at improving clinical outcomes and minimizing variation.

  • In collaboration with physician leadership and the multidisciplinary team, develop clinical pathways.

  • Participate in and lead quality assurance activities (i.e., chart reviews, peer review) and establish standard of practice.

  • Evaluate, diagnose, treat, and manage acute, critical and chronic patients’ illness and diseases within both an outpatient clinic.

  • Obtain medical histories, conduct physical examinations and assess and determine differential diagnosis. Prescribe medication, therapy and treatments accordingly.

  • Interpret and evaluate patient data for the purpose of determining management and treatment of patients. Perform minor surgeries and routine procedures. Consult with physician as appropriate. Orders, performs, and interprets diagnostic studies in the field of clinical specialty trained and determined competent by a physician.

  • Adjust patient care plan by reviewing and monitoring treatments and therapy plans. Counsel, instruct and educate patients on physical and mental health including: diet, preventive health measures, health behaviors, self-care skills, disease, treatment options, and normal growth and development.

  • Provides care coordination, which includes managing a variety of ancillary services, pre-operative plans, discharge planning, provider referrals and medical devices.

  • Prescribe, recommend and regulate pharmacologic and non-pharmacologic therapies across multiple care settings, including controlled medications that are warranted by patient's diagnosis or changing medical condition.

  • Prepare, dictate and sign patient care plans and summaries of patient care. Maintains records according to established standards and guidelines.

  • Provide clinical direction to nursing staff regarding the continued care of patients. Follow through with nursing staff to ensure patients receive recommended care.

  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
