Bethesda Lutheran Services Student Assistance Program Liaison (SAP) in Corry, Pennsylvania
School-based program that provides screening, assessments and collaboration of mental health services for students between the parents, school officials and outside agencies.
Essential Functions of Position:
Collaborate with parents, teachers, school administration, and outside agencies through phone calls, meetings, ESAP meetings.
Assist students in the coordination of needed services (i.e. MA loopholes,
health insurance, mental health services etc.)
Screen and complete assessments on referred students and recommend mental health services.
Coordinate and assist with establishing psychiatric services when needed.
Collaborate with Millcreek Community Hospital on discharge plans.
Make referrals to outside agencies for services.
Provide individual and group counseling.
Complete and submit electronic monthly data tracking spreadsheet to IU5.
Utilize Behavioral Health Works (BH-Works) for initial assessments with middle and high school students.
Attend mandatory quarterly IU5 SAP meetings.
