The Level2 Registered Nurse delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of type 2 diabetes. The Registered Nurse is an integral part of the interprofessional care team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life. The Registered Nurse supports and advocates for people affected by type 2 diabetes to optimize quality care. The Registered Nurse promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.

Primary Responsibilities:

  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice

  • Advocates for and communicates about improved quality of care and outcomes for those living with type 2 diabetes

  • Actively participates in the quality improvement process and adapts practice/process based on quality findings

  • Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes

  • Applies the AADE7 self-care behaviors to educate on and initiate behavior change

  • Engages in lifelong learning and serves as a role model of professionalism

  • Provides quality diabetes self-management education via telehealth visits based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with diabetes

  • Follows the National Standards of Diabetes Self-Management and Support Guidelines

  • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’ assessed needs and goals and promotes successful self-management

  • Documents all individual contacts/visits in the Electronic Health Record and outcomes data base according to the guidelines in a timely manner

  • Collaborates, advocates, and confers other members of the Level2 care team in developing person-centered diabetes plans

  • Evaluates and analyzes continuous glucose monitor data to guide diabetes education and support

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations

  • Participates in meetings, serves on committees, and represents the Level2 nursing department within the enterprise

  • Performs other duties as assigned

Candidate must be available to work Monday-Friday 8:00 am - 5:00 pm with at least one day per week 10:00 am - 7:00 pm.

Required Qualifications:

  • Current and unrestricted RN license in a compact eligible state

  • Certified Diabetes Care and Education Specialist

  • 1+ years outpatient diabetes self-management education and support experience

Preferred Qualifications:

  • Working knowledge of diabetes technology/continuous glucose monitors

  • Basic computer skills including use of Microsoft Office

  • Demonstrated self-sufficient troubleshooting technology problems and working independently

  • Proven excellent verbal and written communication skills

  • Proven highly organized and strong attention to detail

  • Proven solid problem solving and critical thinking skills

  • Proven ability to work with a diverse patient population and have exceptional customer service skills

California, Colorado, Connecticut, Hawaii, Nevada, New Jersey, New York, Rhode Island, or Washington Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
