TITLE OF POSITION: CBS Case Manager (Project Before Family)

SUPERVISOR: CBS Program Coordinator

WORK SCHEDULE: MINIMUM QUALIFICATIONS:

? Must have a Bachelors Degree in Psychology, Social Work, Early Childhood, or a related area OR four years equivalent experience in provision of services to children and families.

? Must have the ability communicate positively and effectively with others, to work independently as well as a productive part of a team.

? Must complete Case Management training approved by the State.

? Must have a dependable vehicle, valid drivers license, and be in compliance with agencys liability carriers requirements.

? Requires use of cell phone during working hours GENERAL RESPONSIBILITIES: Must be able to develop rapport with diverse families of the targeted population, who have children in the home, ages birth to 5. Must be able to function in a strengths-based model of care. Must be able to link families with resources, coordinate appropriate care/service provision, and provide documentation of services provided to family unit. ESSENTIAL JOB FUNCTIONS:

Quantity: Meets billable visit and hour standards as defined by program requirements. Meet with families regularly, demonstrating persistence and dedication when making contact is difficult or unpleasant.

Clinical: Contact with families is strengths-based; All services are delivered in ways that are responsive to family needs and are in accordance with all agency policies and grant specifications; Service provision is guided by a Family Goal Plan developed with the family; Interventions utilize naturally occurring resources when possible and are focused on promoting success/stability within the family system.

Provide intensive community-based and in-home support to families that reduce risk factors and increase protective factors so that young children have improved developmental outcomes and a foundation for school success.

Judgment: Demonstrate sound judgment in crisis or ongoing care/service provision that is in keeping with ethical and practice guidelines.

Documentation: All documentation is completed in a timely manner, is thorough, appropriate, and complies with agency/grant standards; Documentation accurately reflects interventions and family progress toward estab-lished goals.

Leadership: Models respectful, supportive, and professional conduct in all work environments; Informed, responsive, and respectful of racial, cultural, and ethnic differences among families; Demonstrates an awareness of how behaviors and speech impacts family system/others.

Coordination: Work collaboratively with both agency peers and partnering organizations while building a professional network that assists all families being served; interacts with other people and programs helpfully and positively.

Financial: Understands and supports business model needed to financially support family needs and organi-zation structure. OTHER JOB FUNCTIONS:

Exhibit and promote decisions / behavior consistent with the Centers established Vision, Mission, and Values.

Any deviations to the work schedule and all overtime must be approved in advance.
