Major Responsibilities:

The Family Support Navigator (FSN) is an active member in the FLACRA team and provides peer support services to family members of individuals with serious mental illness and or substance use. Under the supervision of the Program Manager the FSN will function as a role model to peers, family members and the community exhibiting competency in personal recovery and use of coping skills; serve as a consumer advocate, providing consumer information and peer support for clients in outpatient and inpatient settings.

Must Have Qualifications:

  • Highschool Diploma or GED equivalent

  • Valid NYS Drivers’ License

  • Lived experience - your own struggles or have supported / cared for someone facing mental health or substance recovery challenges

  • Willing to participate in peer certification training, acquiring CRPA-P no later than 4 months after employment.

    Nice to Have Qualifications :

  • Certified Peer Support Specialist (NYCPS) trained by OMH or OASAS

  • Certified Peer Advocate (CRPA)

Job Requirements:

Duties

  1. Serves as an active member of the client’s treatment team, including participation in team meetings and treatment planning meetings.

  2. Assists clients in development of recovery plan, including crisis plan.

  3. Implements and coordinates recovery plans for individual clients in assigned population group.

  4. Provides case management, resourcing, service collaboration for individual clients as extension of case management services.

  5. Maintains accurate, thorough records on all assigned cases and uses documentation to assist clients in evaluating progress.

  6. Provides advocacy services on behalf of clients.

  7. Informs supervisor, and other members of treatment team as indicated, of issues/concerns related to individual clients.

  8. Anticipates in required trainings to maintain certification.

  9. Knowledge of Crisis Intervention and Early Engagement.

  10. Assist individuals and their loved ones to become engaged in recovery-focused activities through guidance, education, and support regarding addiction and opioid abuse treatment, understanding the progression and impact of addiction, finding support groups, cultivating peer interactions, and engaging in additional treatment and community services.

  11. Cultivates relationships with local addiction prevention, treatment and recovery services, managed care organizations, local substance use disorder councils, coalitions and community stakeholders to assist families with accessing local treatment and support services.

  12. Demonstrates teamwork and an ability to work in a team environment with coworkers and supervisors. Demonstrates the ability to be flexible and cooperative.

  13. Responds appropriately to crisis/emergency situations in order to ensure/secure the safety/wellbeing of the clients.

  14. Assures all documentation is completed in a timely and accurate manner in compliance with agency/OASAS regulations, practices, policies & procedures.

  15. Demonstrates professionalism by providing quality service and care with respect and dignity for the clients and as a representative of FLACRA.

  16. Provides back up support in times of crisis/urgency to other COTI team members

  17. Provides 1 week of overnight on call services in rotation with others.

  18. Tier 2 and 3 staff are expected to model leadership and help support Tier 1 peers.

  19. Perform other duties and functions as directed.
