Applewood Centers Clinical Diversion Specialist in Cleveland, Ohio

Applewood Centers is a private, nonprofit agency providing mental health services, social services and education to children, adolescents and families living in Cuyahoga and Lorain counties. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.

Position Summary: It is estimated that approximately 70% of youth admitted to the Cuyahoga County Juvenile Court Early Intervention and Diversion Center and Domestic Violence Diversion Program (CALM) have experienced some form of mental health concern. 

Applewood Centers has partnered with the Cuyahoga Juvenile Court to provide specialized programming to reduce court involvement as well as decrease the recidivism rate for youth. In the last year, 96% of youth involved with our Diversion programs have had no new legal charges filed.

The Diversion Specialist will ensure the appropriate placement of youth throughout our array of referral and treatment services for the Cuyahoga County Juvenile Court Early Intervention and Diversion Center and CALM Program.  In this role, you will provide clinical assessments based on verbal, written, and face-to-face interviews to determine appropriate level of treatment for youth either referred to the Intervention Center or brought to the program by the police for domestic violence calls. Assessors may explore alternatives such as kinship placement or respite placement for up to three days in addition to respite if deemed necessary to remove youth from the home. In less acute cases, assessors will connect youth to appropriate mental health and prosocial services. When necessary, assessors will complete a youth screener, a safety plan, symptomology screeners and connect the youth and guardian with ongoing services. This program provides services 24/7 and will require on-call phone coverage on a rotational basis to meet staffing shortages.  We are hiring for all shifts! 1st shift schedule is 8a-4p, 2nd shift 4p-12a, and 3rd shift 12a-8a.

Responsibilities Include:

  • Responsible for completion of behavioral health and juvenile justice screeners. Communicate results of assessment and recommended services to referring court/law enforcement staff.

  • Administer, track, and submit outcome measures in accordance with program standards.

  • Responsible for case management and referral tracking and ensuring proper funding of all cases before the case leaves intake.

  • Face-to-face or virtual admission with clients and guardian. Entails completing all required clinical and administrative paperwork. Completion and submission of behavioral health reports and safety plans to referring court personnel. When applicable, it may be necessary to complete Psychiatric Diagnostic Evaluation and Treatment Plans for new admissions to Applewood Centers programs.

  • Coordinate and link with other community behavioral health and substance use providers to ensure continuity of care for open cases.

     

    Qualifications:

  • Minimum Bachelor’s degree in Social Work or related clinical field.

  • Valid Ohio LSW or PC required.

  • 3-5 years’ experience in behavioral health, substance abuse, and/or juvenile justice fields.

  • Experience working with children, adolescents, and their families.

  • Strong clinical skills including expertise in juvenile justice, systemic family therapy, crisis intervention, family education, behavioral interventions, and substance use treatment.

  • Experience in clinical diagnosis and assessment.
