Ohio Means Jobs Cuyahoga County is sourcing Home Health Aides for a local health care provider.
* Position Summary*
Provide personal care to patients as delegated by a professional member of the patient care team. Responsible for paraprofessional aspects of care and patient safety. Functions under the overall supervision of the Paraprofessional Supervisor. Preferred Experience
Experience in caring for the geriatric population in a healthcare related setting preferred, but not mandatory. Must be able to communicate verbally.
Skills
Demonstrates good interpersonal relationship skills. (Training provided training as needed.) Ability to communicate verbally. ABILITY to lift up to 50 pounds unassisted and up to 325 pounds utilizing an assistive device.
Position Responsibilities
- Provide for personal care duties including but not limited to: personal hygiene and care, patient mobility, elimination and nutrition.
- Report changes in the patient's condition and needs in a timely fashion.
- Submit to the organization, completed documentation within the established time parameters.
- Perform all duties within the boundaries of the organizations policies and procedures.
- Provide care to the patient with attention to the safety needs of the patient.
- Perform household duties as designated by the care plan.
- Utilize equipment and supplies appropriately.
- Review workload and assignments as requested by the Paraprofessional Supervisor.
- Take initiative in performance of assigned work, requiring minimum supervision.
- Accompany (not transport) client to appointments as permitted by plan of care.
- Demonstrate a high degree of commitment to customer service and the provision of quality care.
- Exhibits positive interpersonal verbal and written skills.
- Perform all other duties as requested or assigned.
*Training Requirements* Medicare/Waiver aides must attend a minimum of 12 hours of in-services each year and 40 hours of training prior to hire with the remaining hours completed within the first year. PASSPORT/PC aides must attend a minimum of 8 hours of in-services each year and 60 hours of training prior to hire.
