Description: On both Base and Liberty TCC units, we serve positive pressure dependent patients (trach/ventilator dependent, non-invasive/BiPAP dependent) ages infant to adult. Patients on these units have a variety of complex disease processes that affect their airway, lungs, and respiratory mechanics. Some frequent patient populations that are seen on the TCC are Bronchopulmonary Dysplasia (BPD), neuromuscular diseases such as Duchenne's Muscular dystrophy or Spinal Muscular Atrophy, and restrictive lungs disease due to scoliosis or chest wall abnormalities. We train families on the home ventilator, tracheostomy tube care, GTube care and emergency preparedness with taking home a medically complex loved one with technology dependence. On the A7C unit, we manage many other complex pulmonary conditions including Cystic Fibrosis, Asthma, Obstructive Sleep Apnea, Rare Lung Disease, and patients who are in evaluation or have been recipients of a lung transplant. In all three units, we train families on how to provide their child's care needs at home. This includes teaching and education on home ventilators, tracheostomies, airway clearance modalities, medications, feeding regimens, and emergency preparedness. We have amazing care managers and social workers who organize home nursing if needed and other resources for the home. While we are pulmonary medicine, our patients are complex and often have several other diagnoses that cross over many body systems. Schedule: Rotating/Flex (Days and Nights) - 12 hours shifts - Includes holidays and weekends - Call requirement JOB RESPONSIBILITIES * Patient Care- Perform history and/or physical exams on patients presenting for care in area of expertise. Assess and diagnose illnesses, including chronic and acute conditions. Formulate, document and monitor a treatment plan. Order and monitor therapeutic interventions including but not limited to diets, laboratory, radiological and diagnostic tests as deemed necessary by patient history and/or physical exam. Provide clinical consultation, order interdisciplinary consults. Professional practice including but not limited to prescription of medications in accordance with Federal and State Law & Rules and CCHMC policy.
