Catholic Charities of the Archdiocese Service Navigator & Coordinator in Chicago, Illinois

Essential Functions of the position are detailed below, and include any physical requirements below that.1. Works independently and consistently exercises discretion and sound judgment in providing intensive service navigation and coordination services to assigned clients. May work evenings and weekends in order to complete job duties;2. Works to stabilize youth in their current environment, increase their self-efficacy, and assist in their transition to adulthood;3. Provides effective interventions utilizing a client centered, strengths-based, goal oriented process for assessing the needs of an individual and assisting the client in obtaining the required services and meeting service plan goals. Provides crisis interventions as needed and reports all critical issues to supervisor immediately. Creates well written and effective service plans which appropriately match the needs of clients following the standards and guidelines of the funders, contractor and governmental regulations; 4. Makes appropriate referral, monitors client services in the appropriate time frames, seeks supervisory guidance as required and documents services provided;5. Knowledgeable about community resources and links clients to these resources. Follows up on all referrals and progress toward service plan goals. Advocates on behalf of client in order to ensure access to services for which the client is eligible;6. Maintains up-to-date and accurate client files including recording of all significant contacts, referrals and maintaining Agency and funder databases as required by Agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies. Provides timely completion of all necessary documents in case files;7. Participates in all funder required meetings and trainings;8. Actively participates in weekly reflective supervision and is prepared to discuss any issues impacting the clients that may indicate additional services are necessary. Seeks supervisory consultation around issues of abuse, suicide, and other identified risk factors;

Education and Experience Requirements:

Relevant Education:Preferred: Master's Degree in human servicesMinimum: Bachelor's Degree

Minimum: 1 Year experience working with individuals and families in the Austin community
