Adecco Healthcare is searching for a Registered Nurse (Case Manager Experience) to become part of our client's team in Chicago, IL - 60603. This position is direct-hire.

Pay

$80,000-$90,000

Shift

  • Monday-Friday

  • 8am-5pm

  • No on-call

  • No weekends

Additional Details

  • A lot of field work (mileage reimbursement provided).

  • When not in field, will be in Chicago office doing virtual visits or also doing work at home (company laptop provided).

Job Summary

The Registered Nurse (Case Management) role involves a mix of field work and virtual visits. The RN is expected to be self-motivated and require minimal oversight. Key responsibilities include supporting census management, reviewing patient data, coordinating transitions of care and post-discharge planning with healthcare providers, as well as potentially conducting outreach to patients by phone. The RN will play a crucial role in new patient engagement and overall care coordination.

Qualifications

  • IL RN license.

  • BSN degree.

  • At least 5 years RN experience.

  • At least 3 years of care management experience in the health plan, home health, and/or hospice.

  • Case Management experience on the provider or payor side with a focus on patient interaction.

  • Passion for helping underserved population.
