Barry-Eaton District Health Department Health Access Supervisor in Charlotte, Michigan

Position: HEALTH ACCESS SUPERVISOR

Barry-Eaton District Health Department is seeking a full-time Health Access Supervisor *to lead the organization's efforts to connect designated populations to social and health resources in Barry and Eaton County. This role provides leadership and daily oversight to our team of Community Health Workers (CHW). This position is based out of our *Charlotte, MI office.

In the Health Access Supervisor role, you will have the opportunity to:

  1. Provide leadership and support to CHW team through training, mentoring, and case conferencing. Supervise, hire, assign work, train, evaluate performance, provide feedback, and effectively resolve employee relations matters within team.
  2.  Assist CHWs in navigating various service and support systems. Provide resource information, case management and advocacy. Collaborate with local service providers to secure needed services and supports for clients.
  3. Ensure accurate and timely documentation of client encounters, and program outcomes in the Electronic Medical Record (EMR) system in compliance with organizational standards and funder requirements.
  4. Collaborate with internal and external partners, including healthcare providers, social service agencies, government agencies, and community-based organizations, to coordinate services, leverage resources, and address social determinants of health.
  5. Participate in general and programmatic policy and procedure development and implementation. Explain and clarify policies and procedures to employees and the community.
  6. Prepare reports, presentations, and proposals to communicate program accomplishments, challenges, and needs to stakeholders and funders.
  7. Serve as a community representative by attending community coalition and workgroup meetings, making presentations on behalf of the programs, and fostering collaborative relationships with community partners, community resources, health plans, and providers.
  8. Maintain education to be up to date on patient ethics and standards, best practices and interventions, and developments within the field.

Ideal candidates will possess:

Master's degree in Public Health, Social Work, Community Health, or related field

Michigan certification of registration as a Social Worker or Certified Social Worker, preferred

1 year of program development or progressively responsible experience

2 years of experience in a public health setting

1 year of professional Social Work, preferred

1 year of supervision of staff, preferred

Comprehensive understanding of local healthcare systems, social services, and community health resources.

Familiarity with program development, implementation, and evaluation, including budget management and reporting.
