EAC Network, a dedicated not-for-profit agency supporting over 62,888 people through 100+ programs across Long Island and NYC, is seeking a compassionate and skilled Case Manager for our Suffolk Transitional Case Management Program. Join our team and play a key role in empowering individuals as they reintegrate into Suffolk County.

Position:Case ManagerCompensation:$28 per hourHours:Full-time, 35 hours per weekSchedule:2-3 days per week at Riverhead or Yaphank Correctional Facilities (7:30 AM - 3:30 PM) and the rest of the week in the Central Islip office (9:00 AM - 5:00 PM)Location:Central Islip, NYReports To:Program Director

Why This Role is Important:

As a Case Manager, you will be the bridge between incarceration and community reintegration for individuals with psychiatric illnesses returning to Suffolk County. Your efforts will help them access crucial services, reduce the risk of relapse, and support their successful transition into the community.

Key Responsibilities:

  • Client Engagement:Provide comprehensive transition planning and care management for individuals being released from local correctional facilities and NYS prisons, focusing on those with documented psychiatric illnesses.

  • Collaboration:Work closely with the Jail Behavioral Health Unit to identify and support clients, develop integrated reentry service plans, and ensure linkages to post-release services.

  • Service Coordination:Assist clients in obtaining necessary benefits, housing, vocational and educational services, and peer support networks. Collaborate with the Suffolk County Department of Social Services to activate Medicaid and other benefits.

  • Program Management:Enroll and dis-enroll clients in the Medication Grant Program, respond to Assisted Outpatient Treatment unit requests, and ensure timely linkages to community care management.

  • Documentation & Reporting:Maintain detailed client records, conduct toxicology testing if needed, and provide timely reports to supervision and other relevant entities.

  • Transportation:Transport and escort clients to appointments as required.

What We’re Looking For:

  • Education & Experience:Bachelor’s Degree in Social Work, Mental Health Counseling, or Criminal Justice with at least two years of experience working with mentally ill, justice-involved populations.

  • Skills & Knowledge:Strong understanding of treatment services, effective communication skills, and proficiency in computer applications (Microsoft Office, spreadsheets, email, etc.).

  • Licensing & Transportation:Valid NYS Driver’s License, willingness to use your own vehicle for client transportation, and ability to provide proof of insurance and defensive driving certificate within 30 days of hire.
