Schedule: PRN - available to work up to 4 shifts a month, not guaranteed however, may need to cover weekends and holidays

Shift : Day 8:30am - 5:00pm

Both corporate location and facility-based location, also will be going to patient homes. 5020 E. Arapahoe Rd., Centennial CO 80122 is PRIMARY location, back up location is at corporate office, AdventHealth – Rocky Mountain Region 6061 S. Willow Drive, Suite 210, Greenwood Village, CO 80111

The role you’ll contribute (Job Summary):

Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community.

• Assess patient’s physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data.

• Develop and implement patient plan of care and observe outcomes in accordance with social work standards and in collaboration with the interdisciplinary team.

• Manages resources and patient flow through matching clinical competencies with individualized patient’s and unit needs.

• Use quality measures to improve performance and accountability for patient outcomes, patient experiences and safe delivery of care.

Qualifications

The expertise and experiences you’ll need to succeed :

KNOWLEDGE AND SKILLS REQUIRED:

· Organizational

· Verbal Interpersonal

· Customer Relations

· Grammar/Spelling

· Read/Comprehend written instructions

· Follow verbal instructions

· Keyboarding (manual)

· Computer (visual)

EDUCATION AND EXPERIENCE REQUIRED:

· Education: Master of Social Work (MSW)

EDUCATION AND EXPERIENCE PREFERRED:

· 6 months home care/hospice experience

· Two years post Master’s experience in health care or related field

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

· Cardiopulmonary Resuscitation (CPR)

· Current Basic Life Support (BLS) from American Heart Association for facilities with an Automated External Defibrillator (AED), preferred for facilities without an AED

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:

· Licensed Clinical Social Worker (LCSW)
