Meadowbrook Terrace – LPN Supervisor – Full Time Day Shift – 8a-4:30p-Req#7826
Pay Grade-$28-$37.10*
Plans and implements the care of residents, under the direction of the RN, within the assisted living setting. Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by HHAs, LPNs and supportive staff members. Participates in resident and family teaching. Maintains role as resident advocate with a focus on the facilities mission.
Qualifications:
Adherence to NYSDOH regulations
Supervise LPNs
Assist with Case Management (to include progress not follow-up)
Assist with telehealth visits
Assist with medication management oversight
Assist with Policy and Procedure development
Rotating on-call for staff scheduling
Assist with QA audits
Assisting other areas of facility as needed.
Requirements:
Current New York State Licensure as Licensed Practical Nurse.
Current BCLS & First Aid Certifications.
One or more years of previous experience in long term care preferred.
