Case Manager (CV42-2) - Homeless Services Division

We are looking for a Case Manager to join our dynamic and growing team. The successful candidate will report to the Director/Treatment Coordinator and will be a vital part of their Treatment Team.

JOB RESPONSIBILITIES:

  • Use advanced tools to conduct risk/needs assessments and develop service plans for all clients assigned to your caseload.

  • Implement those plans subject to review by the Treatment Team and/or approval of the Director/Treatment Coordinator.

  • Schedule and facilitate case management meetings with clients

  • Facilitate scheduled workshops, programs, and groups addressing clients' needs.

  • Maintain appropriate contact with outside agencies contracted to provide specific outsourced services.

REQUIREMENTS:

  • Strong interpersonal skills

  • Work well in a team.

  • The capacity to achieve an effective balance between set operational policies and procedures and the use of initiative as and when the need arises

  • Possess effective written and verbal communication skills

  • Have an ability to interact appropriately with clientele

  • Basic computer skills will be preferred

  • Must have a valid driver's license and meet VOADV agency standards

EDUCATION:

  • Minimum of a Bachelor's Degree
