Community Resource Specialist

Department Description: This is a great opportunity to work as a Community Resource Specialist in the Social Work Department. You will work as part of a multidisciplinary team within Inpatient Medical / Surgical Units and within several ambulatory clinics. Responsibilities include, but are not limited to:

• Assessing patient to determine their needs and eligibility for services

• Identifying new and existing resources

• Providing information and referrals to hospital and community based programs

• Developing and documenting a service plan for patients/families

• Maintenance of on-line and written resource materials

• Consultation to care team about resources and service plans for patients

• Assist the team in developing safe discharge plans
