As a Program Director at Horizon, you will…

  • Be responsible for oversight of project operations, implementation, evaluation, and QI.

  • Oversee hiring and training processes for new SBIRT staff and delegate program duties to appropriate positions during first quarter following award.

  • Develop a CQI Plan that will document detailed processes for data collection and utilization in alignment with NOFO requirements.

  • Oversee development of program databases; meet with the EHR team to establish program-specific dashboards in the EHR that aggregate and display information on progress related to our measurable objectives, which we will use as key performance indicators for the program.

  • Ensure timely entry of data into SPARS by program staff.

  • Lead school-based meetings/trainings.

  • Oversee school-based SBIRT clinicians to ensure identified screening tools are being utilized according to plan, and results inform follow-up intervention.

  • Ensure Treatment Protocols are developed according to plan and that they are adhered to throughout patient participation in SBIRT.

  • Oversee outreach to communities/organizations in catchment area that serve minority populations inc. persons of color/LGBTQ+ young people to offer SBIRT programming.

  • Ensure that EBPs are being implemented with full fidelity; review/update policies/procedures to ensure that EBPs are consistently applied in the way they are intended.

  • Establish annual monitoring and reporting processes for reviewing EBP implementation to ensure that staff are adhering to established processes without modifications.

  • Oversee development and implementation of partner agreements.

  • Develop and submit behavioral health disparities impact statement.

  • Collaborate to develop a sustainability plan with step-by-step actions to integrate SBIRT into daily care and treatment protocols following grant period.

  • Refine program operations as barriers are encountered and lessons are learned.

  • Continually assess system gaps and correct as needed.

  • Maintain confidentiality of information being processed/stored/accessed for the program.

  • Attend all SBIRT staff meetings.

What makes you a great candidate?

We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:

  • Completed Master’s degree required along with one of the following licensures: LMSW, LCSW, LMHC or CASAC.

  • Experience working with children/adolescents.

  • Ability to coordinate with community stakeholders

  • Experience in evidence based clinical practices

  • SBIRT trained preferred.

Location

This position will be located at 699 Hertel Ave, Buffalo NY.

Hours

This is a full-time position from Monday - Friday.

Compensation

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $69,605 - $75,000 . Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

*Please note: This is a grant-funded position.

Physical demands associated with this position include:

  • Sitting for extended periods, alternating between standing and sitting

  • Mobility required with repetitive wrist, hand, and finger movements

  • Handling light duties, occasionally lifting objects up to 20 pounds
