As a RN at Horizon, you will…

  • Assess patient needs and develops a nursing care plan for care coordination and linkage with community resources.

  • Complete medical assessments for behavioral health and substance use disorder patients, provides integration/health education, and develops a nursing care plan to provide holistic and individualized treatment to address each patient’s needs.

  • Administer IM medications to patients while providing education regarding purpose and use of medication, assessing patients’ presentation compared to baseline, and noting any progress or decompensation in symptoms.

  • Assist patients in achieving their treatment goals through health education. Will provide one-on-one education to patients and provide nursing education through group meetings.

  • Provide phlebotomy and EKG services along with education to patient regarding purpose of testing.

  • Provide a therapeutic environment and fosters a team approach with multidisciplinary staff.

  • Evaluate patient responses to treatment and provides reports to physicians, nurse practitioners, and counselors of both objective and subjective data of patient behaviors.

  • Utilize nursing diagnosis, identifies priorities of care, sets realistic short-term goals and outcomes, monitors progress toward goals and adjusts care coordination accordingly.

  • Implement nursing actions that promote, maintain, and encourage healthy lifestyle behaviors.

  • Work collaboratively with the team and identifies goals, lifestyle and behavioral modifications, and appropriate nursing interventions for each patient.

  • Identify health education needs and knowledge deficits of the patient. Provides appropriate health teaching through multimedia materials, documents health teaching and responses in the group notes.

  • Identify patient needs for community services and provide linkage.

  • Develop professional interpersonal relationships with the members of the multidisciplinary team, patients, and families.

  • Attend and participate in all staff meetings and mandatory in-services. Provides appropriate ideas and suggestions to improve the process or operations of the facility.

  • Demonstrate proficiency and accuracy in the administration of all ordered medications and knowledge of appropriate use of medication and adverse effects.

  • Assist Medical/Psychiatric providers with integration, medication management, and psychiatric visits.

  • Demonstrate proficiency in computer skills and provides accurate documentation for medical records/EMR.

  • Have the ability to follow physician/provider orders and recommendations for medical treatment of patients.

  • Provide a safe environment for patients, staff, and families and considers safety in all interactions.

  • Identify and triages medical emergencies and secures appropriate medical treatment for patients, family, or staff.

  • Attend educational conferences and webinars and seeks out opportunities for professional growth and development including attainment of advanced degrees and certifications.

  • Demonstrate proper procedure and technique during venipuncture, collection, and processing of blood samples and collection and testing of urine toxicology and HCG samples.

  • Be comfortable providing telehealth services.

What makes you a great candidate?

We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:

  • Graduation from a regionally accredited or New York State registered college or university with a Nursing degree - associate degree preferred.

  • Candidates must have a current New York State professional nursing license to practice as a Registered Nurse.

  • 2- 5 years of clinical experience

  • Experience working with mental health and substance use disorder patients preferred.

  • Certificate of CPR/AED Training and First Aid.

Location

This position is primarily at our 3020 Bailey Ave clinic; however some travel is required to cover RNs at other clinics as needed

Hours

This is a full-time day shift position from Monday-Friday.

​Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $37.25 - $42.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
