WILLIAMS COUNTY Case Manager/Service Coordination Facilitator for Williams County Family & Children First Council in BRYAN, Ohio

Service coordination is a process of service planning and system collaboration providing individualized services and supports to youth and families who have needs across multiple systems.  The facilitator is a person who coordinates the service coordination process.  Job functions include:

  • Manage a caseload of Williams County Family & Children First Council (WCFCFC) youth and their families as deemed appropriate by the WCFCFC Coordinator in accordance with the current, approved WCFCFC Service Coordination Mechanism to include, but are not limited to:
    • Ensuring Service Coordination Mechanism components are completed timely, such as Consent for Release & Exchange of Information, Strengths & Needs Assessment, Crisis Plan, and Safety Plan.
    • Assisting family in composing Youth & Family Team membership.
    • Facilitate the development, monitoring, and tracking progress of Plan of Care.
    • Being the lead communicator and keeping lines of communication open with Youth & Family Team, scheduling ongoing/emergency Youth & Family Team meetings.
    • Monthly check-in with the youth and family regardless of Youth & Family Team meeting schedule.
    • Keeping track of and submitting required documentation in the software system.
    • Planning for transition and discharge.
  • Provide referral and linkage to identified supports, such as Parent Advocacy Connection.
  • Accurately document time and activity for purposes of service tracking and billing.

This is a full-time position, working 40 hours per week, 12 months per year.  Benefits include medical insurance, membership into Ohio Public Employees Retirement System (OPERS), paid leave and holidays, and flexible schedule.

Qualifications

The Service Coordination Facilitator requires excellent judgment, direct communication, tact, problem-solving, sense of fun and adventure, flexibility, multi-tasking, and ability to lead group activities.  Additional qualifications include:

  • Associate degree required, bachelor's degree preferred in social work, psychology, sociology or a related field and a minimum of two plus (2+) years proven experience in social work or human services.
  • Must possess a valid driver's license, reliable vehicle and be insurable.
  • Must pass a criminal background check.
