LaSante Health Center Homecare Coordinator in Brooklyn, New York
Lasante Health Center is seeking a Homecare Coordinator to join our team. The ideal candidate will be responsible for checking in on our patients over the phone and facilitating communication with healthcare providers to ensure they receive high-quality medical care.
Responsibilities:
Schedule doctor visits for clients requiring homecare services.
Coordinate with healthcare providers to arrange appointments and obtain necessary forms.
Collect, organize, and maintain accurate medical records
Ensure compliance with regulations and maintain confidentiality
Additional tasks as directed by supervisor
Qualifications:
Customer service experience, particularly in a healthcare setting.
Proficiency in using computers and electronic medical records systems (ECW preferred)
Ability to maintain a friendly and compassionate demeanor when interacting with patients and their families over the phone.
Ability to manage multiple tasks efficiently and prioritize effectively.
