Amergis Hospice LPN in Bronx, New York
The LPN/LVN – Hospice is responsible for providing care,comfort and support to terminally ill patients. The LPN/LVN – Hospice assumesthe responsibility and accountability for the delivery of hospice patient carein various settings. The LPN/LVNdemonstrates the ability to make clinical judgments in an effective andefficient manner with supervision. The LPN/LVN will also demonstrate criticalthinking and performance ability in the coordination of patient care.
Essential Duties and Responsibilities:
Documents the patient’s plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventions.
Identifies and prioritizes actions based on patient care requirements
Performs interventions according to identified priorities,plan of care, and the hospital policies and patient care outcome standard
Provides a safe, comfortable and therapeutic end-of-lifepatient environment
Provides patient and/or family education, guidance andsupport
Documents the patient’s response to nursing and otherinterventions
Uses clinical judgment in evaluation activities to meetpatient care needs of an assigned floor/unit including establishing priorities
Monitors safety and effectiveness of the environment andequipment
Advises grieving family members on coping techniques andcommunity resources • Performs other duties as assigned/necessary
Minimum Requirements:
Current Licensed Practical Nurse/Licensed Vocational Nursefor the state in which the nurse practices
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis' Competency programwhen applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
