Position Summary:**The P2Project (The Pregnancy Prevention Project) at Children’s Aid aims to reduce and prevent the incidence of unintended adolescent pregnancy in its priority areas in NYC by providing young people with the holistic tools to meet their greatest potentials through evidence-based sexuality education, comprehensive reproductive health services, and peer health education. The main objectives of the program are:

  • Provide comprehensive, age appropriate, sexuality health education for adolescents (11 – 22).

  • Provide strategies for young people to make healthy decisions and set goals for the future.

  • Connect youth to reproductive health services.

  • Encourage youth/adult partnerships and effective communication.

  • Create and foster community and organizational partnerships to reduce the incidence of unintended adolescent pregnancy in our communities.

Under the supervision of the Director of Family Planning and Pregnancy Prevention Programs and Nurse Manager, the Clinical Family Planning Health Educator (HE) provides age specific individual health education counseling in a community clinic on topics such as pregnancy prevention including abstinence, birth control counseling, sexual decision-making, STI and HIV counseling, and other reproductive health topics. The HE functions as part of a clinical team that provides comprehensive health services for adolescents. The HE will be responsible for outreach and follow-up efforts for patients at high risk for unintended pregnancy and STIs and will collect and report health education session outcomes for adolescent health sessions. The Health Educator also provides in-reach and outreach to adolescents in CAS programs, local schools, and CBOs through the provision of collaborating with other department health educators to ensure confidential reproductive health care.

Essential Duties:**

  • Provides individual counseling on pregnancy prevention education, contraceptive methods, STIs, HIV pre- and post-tests, and risk reduction counseling to patients and documents all services required.

  • Provides follow-up for patients at high-risk for unintended pregnancy and/or STIs and documents these efforts in the patient chart.

  • Responsible for triaging and gathering health details about the patients, such as checking vital signs which usually comprise blood pressure, body temperature, respiration and heart beat rate, etc.

  • Works closely with clinical and front desk staff to ensure fast track patients are seen by provider efficiently and effectively

  • Facilitates the provision of safe, quality patient care and the promotion of adolescent friendly services

  • Acts as clinical champion for long acting reversible contraception and works with providers and health educators to maximize the percentage of patients leaving with an effective contraceptive method

  • Participates in the process of developing, obtaining, evaluation, and maintaining written and audio-visual health education materials, as directed.

  • Provides trainings for staff members and peer health educators on related sexual health topics relevant to meet the needs of adolescent patients

  • Collects and maintains accurate client data, activity records, and other related statistical information.

  • Assists with enrollment of adolescent patients in the Medicaid Family Planning Benefit Program.

  • Ensures that all patients, visitors, and guests as well as co-workers receive personalized, prompt attention and are treated with receptiveness, dignity and respect, no matter how limited the contact.

  • Triages all adolescent patients and provide addition clinical coverage when needed

  • Performs other related duties as required.

  • Must abide by privacy laws, including HIPAA, etc.

  • In-person collaboration is an essential function of the job

  • Perform other duties as assigned

* Minimum Qualifications:*

  • Certification to practice to be a Medical Assistant in New York State

  • Strong interpersonal and excellent oral and written communication skills

  • Accurate record keeping skills and organizational abilities

  • Computer literacy

  • Must be able to travel to the Bronx and Harlem to work in multiple clinic sites

  • Must be able to work evenings

  • Experience working in reproductive health settings and adolescents a plus

  • Bilingual: English/Spanish a plus

  • Experience working in team settings

  • Bilingual (English/Spanish) preferred.
